Readmission

• PhD Readmission Application. For PhD students applying for readmission. Supplements the University form Application for Readmission—Graduate.
   

Courses/Enrollment

• VIS STD 299 Special Studies Petition. For students seeking approval for enrollment in Visual Studies 299 Individual Research and Study.
• Waive and Substitute Petition (Graduate Students). For students who wish to waive a program requirement (generally because they have completed an equivalent course) or substitute another course for a required course.
   

Fellowships

• University and Department Fellowship Application. For continuing/returning graduate students wishing to apply for a University and Department fellowship.
   

Graduate Programs

Master of Architecture

• Petition to Extend Program. For students who wish to stay an extra semester beyond what has been specified for them.
• M.Arch Thesis Proposal. For M.Arch students seeking approval for thesis topic and committee. The University form Application for Candidacy must also be submitted.
   

Master of Science

• M.S. Thesis Proposal. For M.S. students seeking approval of the PhD/M.S. Committee for thesis topic and committee. The University form Application for Candidacy must also be submitted.
   

PhD Degree

• PhD Exam and Dissertation Proposal. For students seeking approval of the PhD Committee for the exam committee and subjects for examination. Accompanies the University form Application for Qualifying Examination.
• PhD Minor Completion. For certifying that all work for a minor field has been completed.
• PhD Minor Proposal. For securing approval of the student's minor advisor, program advisor, and the PhD Committee for a proposed field study.
   

Student Employment

• ARCH 140 GSI Questionnaire. Accompanies the GSI/Reader Application. 
• GSI/Reader Application. Application for graduate students who wish to work as teaching assistants.
   

Student Travel

• Architecture Conference Travel Request. For reimbursement of certain expenses related to travel to a conference to present a paper.
   

Faculty/Staff Forms

• Advance Written Approval for Overtime. Used to request overtime pay.
• Application Form for Web Server Disk Space. If you are requesting disk space on our web server, please complete this form.
• Check Request. Used to obtain a reimbursement of personal funds spent in connection with official UC business.
• Department of Architecture Time Sheet: [pdf] [xls]. Used by all departmental staff to report their time worked. Print, fill out, and submit to supervisor on the first of the month.
• Departmental Procurement Order. Used to record purchases made using the departmental credit card.
• Form to Request Books. Form for faculty to request that the Environmental Design Library put books on reserve for courses.
• Hire Request. In order to hire a new employee, supervisors and faculty members must submit this form.
• Key Request. Form to fill out when requesting a room key.
• Part-Time Lecturer Application. For people who wish to teach part-time for the Architecture Department.
• Purchase Request. Used to authorize vendors to charge the UC Regents for goods and services purchases.
• Request for Slides from Periodicals. Used by faculty members and GSIs to request that periodicals from the Environmental Design Library be made into slides.
• Student Archives Submission Form. For faculty members to give to students. Must be filled out by students when submitting files for the CED Student Coursework Archives.
  
• Travel Expense Reimbursement Request. Used to document official travel and determine whether or not amounts are due traveller or UC Regents.
